Enrollment Agreement

I hereby confirm my enrollment into a Lifestyle Prescriptions® University degree program.

By signing this enrollment agreement with my digital signature, I agree to the terms and conditions below:

New students may cancel their enrollment with written notice of cancellation within three days of signing the enrollment agreement and receive a full refund. 50% from 3-14 days, after 15 days no refunds are possible. Students with monthly or annual payment plans have the option to cancel their degree programs annually which must be done in writing at least 90 days before the end of the year (based on the initial signup date). Monthly payment plans are still due and need to be paid until the end of the annual term.

I agree to commit and dedicate time attention, and energy to complete all required courses, engage in the discussion groups, and communicate right away with LPU faculty or staff if questions arise or help and guidance are needed. Most importantly, I am enjoying my journey, feeling empowered, raising my skills, and growing as a person and practitioner.
